What to Bring to Treatment

  • Insurance Card
  • Form of Identification (Drivers License, State issued ID, etc.)
  • Prescription Card
  • All Medications (in original bottles)
  • Personal Hygiene Products (toothbrush, toothpaste, razors, sanitary pads/tampons, deodorant)*These items must be alcohol-free
  • Money/Credit Card for Incidentals
  • Cigarettes
  • Casual Clothing
  • Church Clothing – if applicable
  • Shoes
  • Gym Clothing
  • Beach Wear & Beach Towel
  • Sun Screen
  • MP3 Player/ iPod (may not be able to access wi-fi)
  • School Books – if applicable
  • Eyeglasses/Contact Lenses & appropriate accessories
  • Cell Phone (which will not be accessible during treatment)

  • Provocative Clothing
  • Playing Cards
  • Valuables (including jewelry)
  • Clothing or other items that promote sex, drugs or alcohol
  • Anything that contains alcohol
